Egg rolls, cooked chicken, and flour noodles were held at room temperature and measured at 46-52 degrees F, in the frying area, rather than 41 degrees F or below as required. Commercially processed ready to eat food, located in the prep tops, and held more than 24 hours, are not being marked with the date it was opened. Begin practice immediately. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours are not being date marked. Only lidded containers are permitted, and beverages must be stored in a sanitary manner away from food and equipment. Shellstock located in the oyster bar area did not have identification tags attached to the container. ID tag must be kept with the shellstock when removed from original container. Re-organize are where tags are stored. It is also suggested to print copies to live in multiple areas of storage. Several raw animal foods were stored above ready to eat foods in the walk in cooler and beer cooler. Items were voluntarily discarded were needed. Post a food storage poster in both coolers. Clean all listed surfaces and create a cleaning list at a frequency that will keep them clean. All ice in use was disposed of and machine was cleaned immediately. In the walk in refrigerator, raw beef was being stored over ready-to-eat foods, which could allow the meat to drip onto foods and food containers. Corrected on site: Foods were rearranged so that raw meats were in one section below ready-to-eat foods. In the walk in, bowls were being used as scoops, which could contaminate the food. All scoops must have handles, with the part of the handle that touches the hand stored so that it does not touch the food. Corrected on site: Bowls were removed. Dry, soiled wiping cloths were being used to wipe counters on the cook line. Wiping cloths must be maintained clean and either replaced as soon as they are used, or they must be stored in a container of sanitizer, to prevent germs from growing on the wiping cloth. Employees advised on location. Begin either changing out wiping cloths with clean wiping cloths as soon as they are used, or store in-use wiping cloths in a container of sanitizer in between uses. Escolar is being listed on the menu as white tuna, which may confuse customers. Change the menus, including the online menu, to indicate the fish being sold is escolar. A cell phone holder was observed sitting on a cutting board on the cook line, which could contaminate the cutting board. Corrected on site: Cutting board was removed to be washed, rinsed, and sanitized. Employees were advised to store personal items away from food areas. The dishwasher trays have a buildup of grime and need to be cleaned or replaced. The outside dumpster and grease barrels are being stored on dirt, rather than a non-absorbent surface. Move dumpsters and grease barrels to the asphalt. The floor in the back storage area is getting very rough and cracked, which makes it difficult to clean. By June, the back storage area's floor must be refinished to be smooth, non-absorbent, and easily cleanable. SUBMIT THE MATERIAL THAT WILL BE USED TO REFINISH THE FLOOR TO THE HEALTH DEPARTMENT BEFORE PURCHASING IT. The hood lights over the cook line were turned off when the cook line is in use. The lighting in areas where employees are working with food must be bright (at least 540 lux or 50 foot candles). This is to help protect the safety of the food, and the safety of employees working with dangerous equipment such as knives or hot stoves. Corrected on site: The hood lights were turned on. Observed multiple boxes of food stored on the floor in the walk-in freezer as opposed to 6 inches or higher as required. Declutter walk-in freezer immediately. Observed food in the hot holding warmer at the front counter between 110-117 degrees F as opposed to 135 degrees F or above to prevent potential pathogen growth. Warmer temperature gauge was increased during inspection and food temps increased to required temps. Thermometer for ensuring proper temperatures of food was not available during inspection. Acquire immediately and train staff how to use. Food facility does not have available quaternary sanitizer test strips to determine appropriate sanitizer concentration Sample pouch of 15 quaternary test strips provided for sanitizer in use. Large, lidded pan of cheese steak meat was found on stove shelf. Internal temp was 140 degrees F. All foods must be held in hot holding once cooked or cooled to 41 degrees F. Meat was moved to grill top to stay hot. Ensure staff are heating foods to proper temps and holding hot foods at 135 degrees F or more. Beer coolers in both bars were 50 degrees F. All TCS foods must be discarded. Coolers must maintain product at 41 degrees F or less. Sautee bain marie was 47 degrees F. Foods in the unit bottom of the unit were 45-49 degrees F. All TCS foods were discarded. Unit must maintain foods at 41 degrees F or less. Do not store TCS foods in this unit until it is repaired and can maintain foods at 41 degrees F or less. Wing sauce is not stored in refrigeration. The addition of butter to the recipe makes it necessary to store it refrigerated. If you wish to store this as a "shelf stable" product you would have to have it tested to prove that it has a low enough pH level. Discard the current product and store future batches in the walk in. House made dressings, sauces, pasta were over 7 days old. Foods may not be held for more than 7 days. All were discarded. Fish observed thawed in reduced oxygen or vacuum packaging. These packages must be opened to allow oxygen to enter prior to beginning the thawing process. Fish was discarded. Ensure staff open packages before thawing. Sports Bar - no Cl was detected at the final rinse cycle of the dishwasher. Test strips provided but still in plastic indicating that they are not used to check the machine. Do not use the machine until it repaired and Cl of 50-100ppm is detected at the rinse. Sports bar waitress station - clean ice bin daily and clean sink. Clean the all dusty ceiling vents, ceiling tile, and the metal shelf supports in the kitchen. Lack of hot water was found throughout the facility. Must address the lack of sufficient hot water throughout the building. Dept recommends installing several on-demand water heaters at sinks throughout the building. Contact Dept with plan for increasing amount of hot water available for the building before work is begun. Hot water at Main Bar hand sink and main level restrooms did not reach 85 degrees F as required. Hot water throughout the facility is an issue. Water at hand sinks must be minimum of 85 degrees F. Sports Bar floor - Sealant/paint has worn off of bar floor. Floor must be resurfaced to be non-absorbent and easily cleanable. Submit plan for bar floor surface BEFORE installing.
