Bella Trattoria, 366 Ridge Rd. Spring City, February 14. Fail. Thermometers for ensuring proper temperatures of food are not readily accessible. Within 24 hours, prove with proof of thermometers for measuring food temperatures being stored in an easily accessible area of the kitchen for employees handling foods. The following sinks need to be resealed or recaulked to the wall: The 3-compartment sink; The hand sink next to the 3-compartment sink. At the larger dishwasher, the chlorine sanitizer could not initially be detected. Corrected on site: Machine was primed and run several times, and dishwasher chlorine residual was measured at 50 ppm by end of inspection. Use the chlorine test strips daily to ensure the sanitizer is between 50-100 ppm. Clean the following areas: The wall behind the hand sink and prep sink next to the 3-compartment sink; The wall behind the dishwashers, especially the larger dishwasher. Food facility was growing and serving raw sprouts without an approved variance. Corrected on site: Facility will serve only cooked sprouts, or raw sprouts purchased from approved suppliers.

Cciu Learning Center, 1635 E. Lincoln Highway, Coatesville, February 14. Fail. Commercially processed ready to eat food, located in the walk in cooler, and held more than 24 hours, is not being marked with the date it was defrosted. This product is to remain frozen prior to use and has a 24-hour shelf life after defrosting according to its safety data sheet. Product was voluntarily discarded. Ice Machine, a food contact surface, was observed to have mold like growth and was not clean to sight and touch. Do not use until all ice is taken out and unit is cleaned. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil: Door handles and gaskets all cold holding units; toaster; interior shelving of bain marie; interior of true lowboy under flat top. Working containers in three comp sink area, used for storing chemicals, cleaners taken from bulk supplies, were not marked with the common name of the chemical. Chemical was voluntarily discarded. Label all bottles.

Cousin Burgers, 37 Bridge St. Phoenixville, February 14. Fail. The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Food handler not washing hands in between separate tasks. Facility storing food in basement hallway and back room under plumbing lines. Store all food in a separate designated approved area with required floors, walls and ceiling surfaces. Several (3) secondary condiment containers stored unclean on window sill next to food prep sink. Several bus pans of food (Raw Chicken) in the reach in coolers open with no covering. Units out of temperature. Food voluntarily discarded. Label all secondary containers with common food labels. Open bag of dry goods stored on table near food prep sink. Several bus pans with raw chicken wings stored in coolers lacking dates required on all food held longer than 48 hours. Food voluntarily discarded due to temperature. Bain marie not properly working. Repair or replace. All equipment behind bar needs to be anchored to floor. Kitchen cluttered with food,unused equipment. organize and clean kitchen throughout. 2-kitchen cooling units not properly working at time of inspection. Ensure all units at tempt at 41 degrees F or below prior to use. Replace if necessary. Left side door broken off on large 2 door cooler. Dishwasher on but not properly working at time of inspection. Repair or do not use until serviced and in proper working order. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Several used knives stored in between food prep tables. Store in a sanitary, clean and sanitize in between uses. Food prep tables throughout kitchen unclean. Non operational food prep sink filled with used equipment, sponges. Clean and sanitize sink, utilize for designated purpose only. The following need further cleaning and sanitizing: Interior of all reach in coolers; Shelves throughout kitchen; Grease like debris dripping off hood ledge; Clean all sinks and sink surrounds. A bucket of spent cloths was observed on floor near food prep sink in a red bucket. Water turned off to 3 bay sink at beginning of inspection. Due to plumbing issues 3 bay sink was not properly set up. Kitchen hand wash sink filled with used pot and coffee cup. Remove, ensure sink is accessible at all times. Sanitize prior to use. Leaks at faucet base and lines under 3 bay sink causing facility to turn off water. Repair immediately. Water not accessible at food prep sink due to a plumbing issue. Repair immediately. At time of inspection facility storing piles of trash bags and cardboard in basement. Facility immediately took all trash to dumpster. Do not store trash in basement. Soap was not available at the handwash sink in the kitchen area. Paper towel dispenser empty at the handwash sink in the bar. Light out above cooks line. Store all chemical containers in separate designated areas. Label all containers with contents. The CFM on file does not work at facility full time. Facility has 10 days to submit proof of enrollment in an approved CFM course for a FULL TIME employee.

Dairy Queen, 1502 West Chester Pike, #874, West Chester, February 14. Pass. In the walk-in freezer, several trays of ice cream novelties observed uncovered. Keep covered during storage. Several cakes in the customer self-service freezer observed not completely labeled. The labels were provided on-site. Maintain. Spray bottle of bleach sanitizer found with concentration >>200ppm. This was emptied on-site. Use test strips and maintain concentration of bleach sanitizer at 50-100ppm.

Downingtown Pizza, 261 Norwood Rd. Downingtown, February 14. Pass. Observed cheese steaks stored directly on the floor in walk in area, rather than 6 inches off of the floor as required. Product was moved off of the floor. Time in lieu of temperature being used in the food facility to control ready to eat Time/Temperature Controlled for Safety (TCS) foods without written procedures or documentation to verify disposition of food. Post written procedures and document Pizza's daily as replaced.

Hong Kong Chinese Restaurant (new), 318 E. Market St. Oxford, February 14. Fail. Observed several containers of food in the walk-in cooler stored open with no covering. Foods were covered during the inspection. Ensure all food is covered in storage to prevent contamination from the facility. Food storage containers, in the walk-in cooler, is not labeled with the common name of the food. Label all food storage containers and comply at all times. Observed food storage container of raw chicken stored directly on the floor in walk-in cooler, rather than 6 inches off of the floor as required. Food storage container was placed on shelf at least 6 inches above the floor at all times. Partially frozen shrimp observed thawing at room temperature on the drainboard of the 3 bay sink, which is not an approved thawing method- shrimp was less than 41 degrees F and placed into the walk-in cooler. Ensure proper thawing as follows: quick thawed in cooking process; under running water at 70 degrees F; in refrigeration at 41 degrees F or less. Large container of cooked rice in the walk-in refrigerator, was not cooled from 135 degrees F to 70 degrees F in 2 hours and/or from 135 degrees F to 41 degrees F within 6 hours, after preparation. It was reported that the rice was cooked yesterday- internal temperature of cooked rice was observed at 48-50 degrees F; Rice was discarded during the inspection. Cooked rice must be cooled from 135 degrees F to 41 degrees F within 6 hours; comply at all times. Proper cooling procedures in Chinese was provided via email; review with food employees. Observed container of cooked rice in the walk-in cooler in containers greater than 4 inches deep, which is not a proper cooling method. Cool TCS ready-to-eat foods in containers less than 4 inches deep and loosely cover to promote cooling from 135 degrees F to 41 degrees F within 6 hours. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the walk-in refrigerator, is not being date marked. Date mark all ready-to-eat TCS foods today and comply everyday. TCS ready-to-eat foods must be consumed or discarded within 7 days. Observed fried noodles plastic container with a crack on side of container. Discard food container and obtain a food grade storage container. The food facility was observed with excessive food debris accumulation, grime and food splatter on floor, walls, and equipment. Clean floor, walls, equipment surfaces and touch points. Clean up all rodent like-droppings and maintain in a clean and sanitary condition at all times. In addition, clean all fire suppression system piping under cooking exhaust hood canopy including grease drip trays. Observed pots/pans stored inside hand sink. Pots/pans were removed from the hand sink during the inspection. Do not store any pots/pans/utensils in the hand sink as the hand sink must be easily accessible at all times. Rear exterior door of the food facility was being propped open. The exterior door was closed during the inspection. Keep all exterior doors closed to prevent entrance of pests. Observed rodent-like droppings on the floor behind the chest freezers, under dry storage shelving behind walk-in cooler and under the 3 bay sink. No live activity was observed nor were any droppings observed on food contact surfaces. Pest control service is conducted monthly. Contact pest control company to conduct a site assessment for the possible presence of rodents. Provide copy of pest control service report.

Pizza Mia Downingtown, 1192 Horseshoe Pike, Downingtown, February 14. Fail. Chicken in the upper drawer of the drawer coolers next to the fryer ranged from 43-48 degrees F. Cooked chicken breasts and diced chicken were 43 and 44 degrees F respectively. Chicken strips were 48 degrees F which were discarded. Lower the unit temperature or contact maintenance. Note: Unit digital thermometer read 38 degrees F. Maintain all TCS (potentially hazardous) food at 41 degrees F in refrigerated storage. Current CFM has a valid certification, but does not have a CCHD certificate. Application with instructions will be emailed. Please complete and submit within 2 weeks.

Popeye's, 1839 E. Lincoln Highway, Coatesville, Follow-up, February 14. Pass. Wood legs/Concreted block legs/supporting dunnage racks, in the walk in freezer, are not smooth, non-absorbent, corrosion resistant. High pressure hose stored hanging from ceiling tiles and pipes through dish area, with potential to contaminate food equipment and utensils. Remove the hose from its current situation as it could potentially contaminate clean dishes and dry stored goods. Hang it from something it is designed with.

Spring House Spirits, 217 Main St. Parkesburg, February 14. Pass. The Premium Levella refrigerator was observed at 50 degrees F and internal food temperatures were observed at 50 degrees F, rather than 41 degrees F or below. All potentially hazardous food was discarded. Do not use this refrigerator and repair or replace immediately.

St. Thomas More Church, 2101 Pottstown Pike, Pottstown, February 14. Pass. Several spray bottles of cleaners were not labeled. Label all bottles with what is inside. In the back room, working containers of cleaners and other cleaning equipment was stored above or on the same shelf as food equipment (gloves, paper towels for food surfaces, etc). Food and food equipment must be stored separately from cleaning equipment, and cleaning equipment cannot be stored above food or food equipment, to prevent contamination. Corrected on site: Items were rearranged so that food equipment was stored on a separate shelf above cleaning equipment. The food facility does not currently have a supervisory employee with the Chester County Certified Food Manager certificate. An employee has attended an approved training class, but has not sent in a CFM application. Within a week, please turn in the CFM application, the application fee, and a copy of the valid Servsafe certificate.

Sunrise Produce/Oxford BBQ, 193 Limestone Rd. Oxford, Follow-up, February 14. Fail. Food utensils at hot hold cases observed stored in a container of water which is not maintained at 135 degrees F. Water was emptied from the containers and refilled with hot water at least 135F. Maintain in-use utensils in water at least 135F or more at all times. The following internal food temperatures were observed in the hot holding display cases: whole potatoes 125-132 degrees F; wedge potatoes 117-131 degrees F; chicken wings (1 large pan) 112-132 degrees F; chicken wings >135 degrees F were placed back into hot holding at >135 degrees F; chicken wings (4 small pans) 110-126 degrees F; cheese stuffed mushrooms 124-125 degrees F; cajun crab mushrooms 115-131 degrees F; bacon wrapped cheese mushrooms 123-127 degrees F. All food less than 135 degrees F was discarded at the time of this inspection. The ambient temperature of both hot holding display units was observed at >135 degrees F. Ensure proper hot holding at 135 degrees F or more at all times. Do not overstock pans with food product. Monitor internal food temperatures to ensure food is maintained at 135 degrees F or more. Both hot holding display cases have integral digital thermometer displays however it is recommended to provide both display cases with extra thermometers to monitor equipment temperatures. Cooked pork ribs which is a Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the Turbo Air refrigerator, was not date marked. No staff could verify the date of preparation for the cooked pork ribs therefore the pork ribs were discarded into the indoor trash can during this inspection. Upon return to the front food preparation area, the person in charge was observed standing by the trash can and when asked if he was removing the pork ribs from the trash he indicated "yes" and that he was going to take the ribs home for personal consumption. After speaking with a supervisor, it was decided the pork ribs must be discarded. Person in charge was followed to the outside dumpster to verify the pork ribs were discarded.

Bombay Dhabba, 490 Lancaster Ave. Malvern, Follow-up, February 13. Pass. Observed gravy at 43 degrees F as opposed to 41 degrees F or below to prevent pathogen growth in the walk-in refrigerator. The gravy was discarded during inspection by person in charge. Gravy must be cooled to 41 degrees F or below within 6 hours in smaller containers without a lid to allow heat to escape. Utilize thermometer to monitor temperature throughout cooling process.

The following structural areas need to be repaired: Broken cove base across from the ice machine; Broken cove base next to the 3-compartment sink; FRP across from the ice machine. Longhorn Steakhouse #5249, 1110 Towne Square Rd. Pottstown, February 13. Fail. On the cook line, one of the hot water utensil wells was measured at 130 degrees F, rather than 135 degrees F or above. Corrected on site: Utensils were removed to be cleaned, and utensil will water was replaced and reheated. Hot water was measured at 137 degrees F by end of inspection. Cooked onions being reheated for hot-holding were measured at 126 degrees F after reheating, rather than 165 degrees F or above before being hot-held at 135 degrees F or above. Corrected on site: Onions were reheated to 165 degrees F. Foods in the salad dressing cold wells were measured at between 47-50 degrees F, rather than 41 degrees F or below. TCS foods (ranch dressing, blue cheese dressing, and shredded cheese) were discarded. Unit must be repaired to hold foods at 41 degrees F or below. In the meantime, salad dressings will be held in the nearby under counter refrigerator. The bar sink needs a container of quat test strips kept in the bar area, so that staff can test sanitizing solutions daily to ensure they are between 200-400 ppm. Corrected on site: Quat test strips were brought to the bar area. Clean and sanitize the following pieces of equipment within 24 hours: The bread warmer drawers, especially the unused top section; At the soda dispenser, the area around and behind the nozzles; The inside of the ice machine, especially the front of the ice chute. Clean the following areas within a week: The ceiling tiles above the dishwasher; The area around the outdoor grease container. Employee cell phone was observed stored on the same shelf as seasoning. Personal items must be stored in a designated area away from foods and food-contact surfaces. Corrected on site: Seasoning was removed, and cell phone was placed where it could not potentially contaminate food or food-contact surfaces.

Persis Indian Grill, 541 Wellington Square, Exton, Follow-up, February 13. Pass. Ware-washing sink does not have basins large enough for their largest piece of equipment. Pots are still being used that cannot be fully submerged in the 3 compartment sink or used in the dishwasher. Will contact and work with management to come to a solution.

Taqueria Los Juarez, 86 Pennsylvania Ave. Avondale, February 13. Pass. The handwash sink did not have water at a temperature of at least 85 degrees F. Water heater was not turned on when inspector arrived though the truck was in operation. Staff turned on the on-demand heater and water over 110 degrees F was supplied. Ensure water heater is turned on at the start of each day.

West Chester Golf & Country Club, 111 W. Ashbridge St. West Chester, February 13. Pass. Scoop being stored in the ice machine with handle buried beneath the ice. This was at the bar area. Get an holder for the ice scoop. Observed frozen food stored directly on the floor in freezer area, rather than 6 inches off of the floor as required. Products were re-arranged and corrected. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 10 ppm, and not 50-100 ppm as required. Work order put in for machine. Ice Machine, a food contact surface, was observed to have food residue and was not clean to sight and touch. Remove Ice and clean today. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean the following surfaces: fan guards in walk in cooler; shelving in walk in cooler; cans and all dry storage in basement. The hot water used for sanitizing in the mechanical warewashing machine did not reach 120 degrees F in the bar area. Correct immediately and only use machine in kitchen until it is functioning. Three compartment sinks will not reach a temperature of 110 degrees F. Discontinue use until it is repaired. Soap was not available at the handwash sink in the bar area. Sink intended for use was not stocked with soap. The food facility does not maintain Certified Food Manager records as required. Chef has active servsafe. Acquire Chester County certified food manager.

Carniceria Camargo, 207 Magnolia St. Kennett Square, February 11. Fail. One door grab and go refrigerator was 50 degrees F. Yogurt in refrigerator was 51 degrees F, hotdogs 49 degrees F and ham 49 degrees F. All TCS foods were discarded. The thermostat was lowered and air temp was 38 degrees F at end of inspection. Cactus salad, salsa and open lunch meat were not date marked with date made/date opened. Ensure staff date foods and use within 7 days. Frozen meats in the grab and go case were not labeled. Ensure staff label all meat. Clean the shelves next to the 3 bay sink. Floor tile in the back half of the kitchen (from kitchen door entrance to walk in area) are broken or missing. This area of the floor must be repaired so that the floor is non-porous and cleanable. Correct within 6 months. A door sweep is needed on the back door. Large gap observed between door and threshold. Facility was found drying beef for jerky. This procedures is not approved. Must cease jerky making immediately. Jerky making requires a HACCP plan. This plan outlines all procedures and safeguards taken to ensure the food is processed properly. Current procedure would not be adequate. Information regarding jerky making and HACCP plan info will be emailed. Until procedures and HACCP plan is approved, jerky cannot be made.

Metal stem thermometer observed being stored in a sanitizing solution in between uses, which is not an approved method of storing in-use utensils. In-use utensils must be stored in an approved manner. Thermometer removed from the sanitizer and placed on a cleaned, sanitized surface. Food Facility did not have correct bleach/chlorine at time of inspection to sanitize food contact surfaces. Sanitizer was purchased by end of inspection. Continue to use every day and sanitize all food contact surfaces through day. Capaccino machine, a food contact surface, was observed to have food residue and was not clean to sight and touch. Clean thouroghly. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Clean the following areas: Shelves in walk in cooler; ceiling and fan gaurds in walk in cooler; trash areas in coffee supply island; bottom shelf in deli display case. The food facility does not employ a certified employee as required. An employee has attended an approved training class, but has not applied for Chester County Health Department Certification. Class is scheduled for 3-10-25.

Dressler Estate, 114 E. Lancaster Ave. Downingtown, Opening, February 11. Pass. Please obtain a CFM within 90 days. Once the test is passed, please apply for Certified Food Manager Certificate online or print the CFM Application and return with a ServSafe copy and the fee by regular mail.

Elite Nutrition (RG), 407 Market St. Oxford, February 11. Fail. Observed excessive ice accumulation inside the chest freezer- defrost. No hot water was coming out of the hot water side of the mop/hand sink in the restroom. The hot water faucet was repaired during the inspection- maintain in good repair at all times. Clean up all cock roach remains and maintain in a clean and sanitary condition at all times. Food facility inspection indicates evidence of cock roach activity on floor behind chest freezer, floor to the right of the 3 bay sink and inside some cabinets, but facility does not have a pest control program. It is recommended to remove the chest freezer from the facility. It was reported that a pest control operator is scheduled to conduct a service today. Provide copy of pest control operator service contract and copies of all pest control service reports. No cockroach activity was observed on food contact surfaces. Food facility operator is applying Raid Ant & Roach spray in the facility to abate cock roach activity. Do not use pesticides in the food facility. Pesticides may only be applied in a food establishment by licensed pest control operator.

The handwash sink in the back of house area was blocked by flour bins and not accessible at all times for employee use. Corrected on site. Plumbing system not maintained in good repair. Spray wand to left of mechanical dish machine missing spray wand cap. Provide new spray wand cap to prevent water from discharging at 90 degrees F. Top of dish machine manifold leaking. Eliminate leak. Soap was not available at the handwash sink in back of house. Soap dispenser to be installed. Provide a push bottle until installed. Soap was not provided at coffee cafe hand sink. Paper towel dispenser empty at the handwash sink on the make/servery line. The light intensity in dry storage area is not at least 10 foot candles, repair/replace light fixture not working.

This product is dispensed via automated dispenser system. Do not use the automated sanitizer dispenser system until it has been serviced and is dispensing sanitizer at the proper concentration. Corrective Action: Quaternary ammonia sanitizer tablets (Steramine) and test strips are available. Ensure proper sanitizing solution concentration at the 3 bay sink between 200ppm-400ppm. Plumbing system not maintained in good repair - observed water leaking at the hand sink in the upstairs ladies' restroom in the upstairs bar area. Make necessary repairs.

Chartwells @ West Vincent Elementary School, 2750 Conestoga Rd. Chester Springs, February 10. Fail. In the laundry/mop room, part of the caulk sealing the sink to the wall is degraded or missing. Within a week, reseal the mop sink to the wall, and provide a picture. Rodent droppings were observed in the laundry & mop room, especially near gaps in the wall around the area where the water supply for the laundry machine comes in. To address this: Clean all new rodent droppings as soon as they are observed and maintain all surfaces of the room in clean condition; Completely seal any gaps leading into the wall, especially the gaps around the area where the water supply comes in; Continue having the licensed pest control company pay extra attention to this area until the situation is controlled. Within a week, send pictures to show the gaps in the wall have been sealed.
